I (the Client/Participant) am responsible for checking with my doctor to clear this Outdoor Therapy activity prior to booking a session.

Activities may include: walking in nature, sitting in nature, hiking up & down steep hills, and unstable paths/walkways, hot or cold weather, rain, unexpected encounters with wildlife and/or other people.

I (the Client/Participant) understand that meeting in a public location for my therapy session means that Therapist Jessica Hernandez cannot guarantee 100% confidentiality but will do her best to keep the conversation private.

I (the Client/Participant) am responsible for letting Therapist Jessica Hernandez know what activities I am comfortable with during my Outdoor Therapy Session.

I (the Client/Participant) understand the inherent risks associated with attendance and participation in Outdoor Therapy and the possible injuries that could be sustained by participants. I knowingly and voluntarily assume these risks and full responsibility for any personal injury or illness, including death, that may be sustained by me, or any loss or damage to property owned by me, as a result of — or in any way arising out of — my attendance and participation in this Outdoor Therapy Session during the Outdoor Therapy Dates, or in travel to and from Outdoor Therapy.

I (the Client/Participant) acknowledge that I am solely responsible for medical or other costs arising out of any bodily injury, illness, or property damage or loss sustained by me through my voluntary participation in this Outdoor Therapy Session.

I hereby agree that in consideration of Therapist Jessica Hernandez allowing me to participate in the Outdoor Therapy Session, I do for myself and my heirs, executors, administrators, and assigns hereby release, waive, and forever discharge Therapist Jessica Hernandez, its owner, officers, agents and employees (hereinafter referred to as “Releases”) from any and all liability, claims, demands, actions, or causes of actions arising out of or related to:

1. Any injury or illness, including death, that may be sustained by me or which I may cause during the course of my participation in the Outdoor Therapy Session, or

2. Any loss or damage to my property, while participating in the Outdoor Therapy Session and any activities incidental to participation in the Outdoor Therapy Session.

I (Client/Participant) hereby agree to indemnify and hold harmless the Releases against all liability for any personal injury or illness, loss or damage to property, or costs, including court costs and attorneys’ fees that may result from or arise out of my participation in this Outdoor Therapy Session.

I (the Client/Participant) understand that I am responsible for giving 24+ hours notification if I desire to cancel this Outdoor Therapy Session to avoid being charged the full fee.
